Required Athletic Forms

Prior to participating in Upper School athletics, all students must receive a physical examination and obtain formal approval from a physician indicating that the student has medical clearance to participate in high school athletics.

Forms must be completed, signed and turned in to the Athletics Office BEFORE a student can participate in practice or competition.

Required forms are:

  1. Physical Evaluation by M.D., Parental Consent to Participate with Health History

  2. Authorization to Travel

  3. CIF Ethics

  4. CIF Concussion Policy

  5. Parent/Guardian Communication Guidelines

Please download, print and complete the forms. Submit signed forms to the Athletics Office in person or via email to Niki Dehner at ndehner@francisparker.org.

Note: Francis Parker School athletics is no longer using the CareFlow system.
