Upper School Athletics Physicals and Required Paperwork

Mandatory Upper School Athletics Paperwork
In addition to a physical examination, all student-athletes and their parents/guardians must fill out mandatory paperwork and return it to Niki Dehner in the Athletics Office by September 5 in person or via email. The deadline is Sept. 5 for all seasons.

All Athletics forms, except the Athletic Preparticipation Physical, will be completed with a digital signature via OnCampus. If your student is registered for a sport next year, you and your student will be receiving notification to complete the forms via OnCampus approximately 1 week prior to your start date and to be submitted no later than the first day of fall practice or first day of school (for all winter and spring sports), whichever comes first.

The physical evaluation form can be found here. All other required paperwork will be available through OnCampus. Students and parents should sign them digitally. There are six forms for parents to sign and four for students. For instructions, click here.
