Mandatory Athletics Paperwork In addition to a physical examination, all student-athletes and their parents/guardians must fill out mandatory paperwork and return it to Niki Dehner in the Athletics Office by September 5 in person or via email. The deadline is Sept. 5 for all seasons.
The physical evaluation form can be found here. All other required paperwork will be available through OnCampus. Students and parents should sign them digitally. There are six forms for parents to sign and four for students. For instructions, click here. Physical Examinations All student-athletes participating in a sport during the 2018-2019 season (fall, winter, and spring) MUST complete a physical examination by September 5.
Onsite Physical Examination: Students can come to the Athletics Office on July 30 beginning at 5 pm to complete their physicals. A physician will be onsite to complete the examination for a $20 fee.

Please note: Students in grades 9 and 11 need to bring their laptops to the physical for Baseline Concussion testing
