Athletics Physicals and Required Paperwork

Physical Examinations
All student-athletes participating in a sport during the 2017-2018 season (fall, winter and spring) MUST complete a physical examination by September 5.  

Onsite Physical Examination:
Students can come to the Athletics Office on August 1 to complete their physicals if they have not done so already. A physician will be onsite to complete the examination for a $20 fee.

When: 5:30 pm for Grades 9* and 10
           6:30 pm for Grades 11* and 12

*9th and 11th graders need to bring their laptops to the physical for Baseline Concussion testing

Mandatory Athletics Paperwork
In addition to a physical examination, all student-athletes and their parents/guardians must fill out mandatory paperwork and return it to Niki Dehner in the Athletics Office by September 5 in person or via email. The deadline is Sept. 5 for all seasons.
