Reimbursement Process

Click here to download the PA Reimbursement Form. Requests should be submitted within one month of the event. At the Linda Vista Campus, the request forms are located in the PA Treasurer’s box (located in the Mail/Copy Room in the Administration building). At the Lower School, the requests are located in the file box to the right of the fireplace in the office.

  • Reimbursement Request Instructions:

    1. Check the Request for Reimbursement line.
    2. List your name, contact telephone number and e-mail address.
    3. Indicate the name the check should be made payable to and the address where the check should be mailed.
    4. Indicate the appropriate account to be charged.
    5. List the activity and the date of the activity.
    6. Have your Chairperson sign the request.
    7. List the item(s) for which reimbursement is being requested. Original receipts should accompany the request when at all possible. (Copies will be accepted under certain circumstances)
    8. Submit the request. Place the completed form in the PA Treasurer’s box at the Linda Vista Campus. For the Lower School; use inter-campus mail and address to the PA Treasurer.

  • Check Request Instructions

    Check request forms are available at both campuses. Please use only the 3-part form. This request is generally used when a check is needed prior to an event (for example when a deposit is required). At the Linda Vista Campus, the request forms are located in the PA Treasurer’s box (located in the Mail/Copy Room in the Administration building). At the Lower School, the requests are located in the file box to the right of the fireplace in the office.

    1. Check the Check Request line.
    2. List your name, contact telephone number and e-mail address.
    3. Indicate the name the check should be made payable to and the address where the check should be mailed.
    4. Indicate the appropriate account to be charged.
    5. List the activity and the date of the activity.
    6. Have your Chairperson sign the request.
    7. List the item(s) for which the advance it being requested. An invoice or similar documentation should be attached.
    8. Submit the request. Place the completed form in the PA Treasurer’s box at the Linda Vista Campus. For the Lower School; use inter-campus mail and address to the PA Treasurer.

      The school has scheduled check runs on the 15th and 30th of each month. Completed requests should be submitted at least 5 days prior to the check run to allow time for additional signatures. Special checks will be cut only in extreme, time sensitive circumstances. Please contact the PA Treasurer or the PA President to arrange to have the request expedited.
